Office policies

We do not provide primary care services. We provide in-depth consultations focusing on the evaluation, treatment, and prevention of chronic disease in adults and children.


Cornerstone Functional Medicine is not a replacement for your primary care physician. Dr. Lewis works as a functional medicine consultant in conjunction with your current health-care team to assist you in understanding why you have become ill and to expand your diagnostic and treatment options to optimize your health. In order to be one of our patients, you are required to have your own primary care provider who will address your acute health care problems, immunizations, routine health care screenings, and annual physical exams. Your current health care providers will continue to prescribe your current medications which they have initiated for as long as you have need of them.




Our consultations are not for treating emergent or acute medical conditions.


If you have a medical emergency, an acute illness, recent trauma, chest pain, or other acute medical conditions, then conventional medical care is the right solution for you. With many years of experience as a family physician and emergency department physician Dr. Lewis can attest to the great value these services render for acute medical problems. Our practice has the goal of helping you to minimize your need for acute care and emergency services. In the event of an urgent or emergent condition either contact your primary care provider, be seen in an urgent-care facility, go to an emergency department or call 911.




We provide telemedicine consults.


Telemedicine Policy under COVID-19 High Risk Conditions Dr. Lewis normally prefers that the first office consultation take place in his office; however, in order to reduce the risk of transmitting the COVID-19 infection, until further notice all visits can be handled through telemedicine by video calls or phone calls unless the benefits of an in person exam outweigh the risks. When the risks of COVID-19 are significantly diminished an in office follow up visit is recommended. Telemedicine Policy when there is low risk for COVID-19 transmission When the risk of contagious disease returns to a low level we will schedule your first consultation on-site in the office with the option of follow up visits being provided by either video calls or over the phone. Because Important information can be obtained through doing a physical examination, Dr. Lewis prefers to see you in our office for the first consultation visit when conditions allow this to take place in a low risk setting. Under extenuating circumstances (poor traveling conditions, sickness, etc.), you could have your first consultation visit over the phone or by videoconference, with your second consultation visit taking place in our office. After your in-office consult, many of the follow-up visits can be handled through telemedicine. For those who desire visits for more than one year we advise that you be examined in our office once a year.




Insurance Benefits: We are an out-of-network provider.


Most insurances provide their members with out-of-network benefits for out-of-network providers. Cornerstone Functional Medicine LLC is considered an out-of-network provider for all insurances. Please check with your insurance provider to determine your out-of-network coverage benefits.




We can not see Medicare or Medicaid patients


Unfortunately, at this time we cannot see patients who have either Medicare or Medicaid, even those who desire to be seen on a self-pay basis.




We provide a free initial 15-minute phone consult.

How do I become a new patient?


If you are interested in seeing Dr. Lewis, he would like to have a free 15-minute phone consult with you.

But first, it is helpful to Dr. Lewis for you to complete this brief online functional medicine health assessment before your 15 -minute phone call with him. Once you have completed the online functional medicine health assessment our office will contact you within the next few days to schedule your free phone call with Dr. Lewis.

Please leave a message on the answering machine with your name, phone number, and good times to call you back if the office is closed. Our office does not send or receive text messages.

How do I get to your office?


When coming from Titusville: Head south on Route 8 for about 3 miles. After you pass Fairview Road you will see a cell phone tower on your right. Turn right onto Sopher Road about 100 feet beyond the tower. Drive for 0.8 miles on Sopher Road during which time you will pass one driveway on your right. Turn at the second driveway on the right at 233 Sopher Road, where you will see the signs identifying our practice. The office is located at the end of the driveway on the right-hand side where you will see another sign identifying our office. When coming from Oil City: Head north on Route 8 for 11 miles. After you pass Buxton Road you will see a cell phone tower on your left. Turn left on Sopher Road about 100 feet before you reach the cell phone tower. Drive for 0.8 miles on Sopher Road during which time you will pass one driveway on your right. Turn at the second driveway on the right at 233 Sopher Road, where you will see the signs identifying our practice. The office is located at the end of the driveway on the right-hand side where you will see another sign identifying our office.




Our office fees


Our fee schedule is as follows:

The preliminary 15-minute phone call: Free

The first consultation visit (The initial intake): $500 (due when scheduling your appointment)

The consult fee includes the 60-75 minutes spent directly with Dr. Lewis as well as his time before and after your visit reviewing your functional medicine analysis, initial intake forms, and any submitted medical records, and developing your initial diagnostic evaluation and preliminary treatment plan.

The second consultation visit (Lab review and treatment plan): $500 (due when scheduling your appointment)

This fee covers the 60-75 minutes spent directly with Dr. Lewis as well as his time prior to your appointment analyzing the results of your lab studies and fine tuning your personalized treatment plan.

The third consultation visit (Follow up): free 30-minute visit*

*If scheduled within two months of the second consultation, there is no fee for the first 30 minutes of your third visit. This visit usually lasts 30 minutes. However, if more time is needed, there will be a charge of $50 for each additional 10 minutes.

If the visit is scheduled more than 2 months after the second consultation, the appointment fee is $150 for every 30 minutes.

Subsequent follow-up visits: $ 150 for every 30 minutes. These fees are paid at the time of your visit.

These follow-up visits are charged at a rate of $50 for every 10 minutes. These appointments usually last between 30-60 minutes depending on the complexity of your situation and your particular needs.

Phone calls and email correspondence:

Please feel free to contact our office by phone or email with any concerns or questions which you may have. Our nurse will discuss your situation with Dr. Lewis, and either the nurse or Dr. Lewis will respond to your questions by phone or email. There is no charge for phone calls or emails with the nurse. If Dr. Lewis needs to personally respond to your concerns by email or phone, the fees are $30 per email and $5 per minute for phone calls.

Functional Medicine Health Coaching: $30 for every 30 minutes.

The first visit is scheduled for 60 minutes and subsequent visits are usually scheduled for 30 - 60 minutes depending on your preference.

An investment in your health now can pay long-term dividends through both improving the quality of your life as well as reducing future expenses for chronic disease in which the underlying issues have never been adequately addressed and/or could have been prevented.

How do I contact the office with questions?


  1. In general, medical questions are best answered during appointments.
  2. Established patients can contact us by email through our secure online medical portal to which you will have access. Issues which cannot be fully addressed through our nurse may require a phone consult or email correspondence with Dr. Lewis.
  3. For prescription refill requests please use the patient portal.

What are your office hours?


Our current office hours are limited and are not necessarily the same every week. Please feel free to call or email at any time. Please leave a message on the answering machine if the office is closed. We review the email and voice mail regularly and will usually get back to you within 48 - 72 hours or sooner.




What if I can't afford the consult fees?


Whether or not you decide to become one of our patients we would encourage you to attend Dr. Lewis' teachings on how to apply the principles of functional medicine to your life. Most chronic disease arises out of factors which we can control through our lifestyle choices and our environmental exposures.
